Here is How to Submit Nominations for the Foundation Awards
Beginning with the 2018 award cycle, the Foundation changed to a single nomination form for all of the awards, in addition to a 1200-word nominating essay and a biography or curriculum vitae of the nominee. The awards packet, containing explanations of all the awards and the one-page nomination form, can be found here. The nominating form is a form-fill pdf. Please fill it out, save it to your computer, and send it by email or post, along with the nominating essay and a biography or curriculum vitae.
